End of Life Care

For patients who are facing end of life, and hence often require more medical input, at Church Stretton Medical Practice we have a named liaison adminstrator with direct dial telephone number for patients and their carers to use. The administrator's role is to offer fast-track prompt access to the surgery for patients/staff for whatever they require without needing to go through the switchboard - be it a home visit, doctor's appointment, prescription or any other assistance which may be needed.

These links all come from trusted resources but if you are unsure about these or any other medical matters please contact your doctor or pharmacist for advice.
